At Crossroads Veterinary Clinic, we get a ton of interesting questions from pet parents. Below are some common FAQs that might help answer any questions or concerns. Please feel free to call us at 920-749-9400 for any other concerns you might have about your pet.

Where is your clinic located?
Crossroads Veterinary Clinic is located at W5493 County Road KK (Calumet Street) just east of the Appleton city limits. We are about ½ mile east of the County Road N and Calumet Street (County Road KK) intersection and across the street from Schmalz Landscaping. Our facility is located at the end of a private drive, which can be deceptive for first-time visitors. The frontage road entrance is located between DC Auto Repair and Kidzland 2 Child Care Center on the south side of the road. After turning onto the frontage road, take a sharp right (west) turn to our driveway.
What do I do in the case of an emergency and your clinic isn't open?

If you have an immediate EMERGENCY outside of our regular business hours, please call Blue Pearl Pet Hospital – Fox Valley at (920) 993-9193, and they will be happy to assist you. They are located at 4706 New Horizons Blvd near the Fox River Mall, not far from the corner of Casaloma Drive and Prospect Avenue.

Do you offer any payment plans?
Unfortunately, we do not offer any payment plans. We request that you pay for services provided during your pet’s visit. We accept major credit cards and provide you with any information you need to submit claims to your veterinary insurance plan. Many of these plans can help cover routine and emergency services.

We accept cash, Visa, MasterCard, Discover and American Express

Can I call and have you fax a copy of my pet's rabies certificate and proof of vaccinations to the kennel where my pet will be staying?

We’d be happy to send proof of vaccination to your pet’s kennel. Just let us know the fax number. You can also access your pet’s vaccine record at any time using your PetDesk app!
